== Occurrence == Trace amounts of plutonium-238, plutonium-239, plutonium-240, and plutonium-244 can be found in nature. Small traces of plutonium-239, a few parts per trillion, and its decay products are naturally found in some concentrated ores of uranium, such as the natural nuclear fission reactor in Oklo, Gabon. The ratio of plutonium-239 to uranium at the Cigar Lake Mine uranium deposit ranges from 2.4×10−12 to 44×10−12. These trace amounts of 239Pu originate in the following fashion: on rare occasions, 238U undergoes spontaneous fission, and in the process, the nucleus emits one or two free neutrons with some kinetic energy. When one of these neutrons strikes the nucleus of another 238U atom, it is absorbed by the atom, which becomes 239U. With a relatively short half-life, 239U decays to 239Np, which decays into 239Pu. Finally, exceedingly small amounts of plutonium-238, attributed to the extremely rare double beta decay of uranium-238, have been found in natural uranium samples. Due to its relatively long half-life of about 80 million years, it was suggested that plutonium-244 occurs naturally as a primordial nuclide, but early reports of its detection could not be confirmed. Based on its likely initial abundance in the Solar System, present experiments as of 2022 are likely about an order of magnitude away from detecting live primordial 244Pu. === Discontinuation syndrome === SSRI discontinuation syndrome has been reported when treatment is stopped. It includes sensory, and gastrointestinal symptoms, dizziness, lethargy, and sleep disturbances, as well as psychological symptoms such as anxiety/agitation, irritability, and poor concentration. Electric shock-like sensations are typical for SSRI discontinuation. Withdrawal symptoms can occur when this medicine is suddenly stopped, such as paraesthesiae, sleeping problems (difficulty sleeping and intense dreams), feeling dizzy, agitated or anxious, nausea, vomiting, tremors, confusion, sweating, headache, diarrhea, palpitations, changes in emotions, irritability, and eye or eyesight problems. Citalopram doses should be reduced gradually when treatment is finished.
Proglumide, sold under the brand name Milid, is a drug that inhibits gastrointestinal motility and reduces gastric secretions. It acts as a cholecystokinin antagonist, which blocks both the CCKA and CCKB subtypes. It was used mainly in the treatment of stomach ulcers, although it has now been largely replaced by newer drugs for this application. An interesting side effect of proglumide is that it enhances the analgesia produced by opioid drugs, and can prevent or even reverse the development of tolerance to opioid drugs. This can make it a useful adjuvant treatment to use alongside opioid drugs in the treatment of chronic pain conditions such as cancer, where opioid analgesics may be required for long periods and development of tolerance reduces clinical efficacy of these drugs. Proglumide has also been shown to act as a δ-opioid receptor (DOR) agonist, which may contribute to its analgesic effects. However, other studies show that proglumide has low affinity to the μ-opioid receptor (MOR) and the κ-opioid receptor (KOR) (13% of MOR and 17% of KOR occupancy at 100 μM), but no affinity to DOR. It is questionable whether this is clinically significant, since the concentration they used was very high, and the occupancies were low even at that point. Proglumide also works as a placebo effect amplifier for pain conditions. When injected visibly to a subject, its analgesic effect is bigger than a similarly administered placebo.
Dimethyl fumarate (DMF) is the methyl ester of fumaric acid. Dimethyl fumarate combined with three other fumaric acid esters (FAEs) is solely licensed in Germany as an oral therapy for psoriasis (brand name Fumaderm). Since 2013, it has been approved by the U.S. Food and Drug Administration (FDA) as a treatment option for adults with relapsing multiple sclerosis (brand name Tecfidera). In 2017, an oral formulation of dimethyl fumarate (brand name Skilarence) was approved for medical use in the European Union as a treatment for moderate-to-severe plaque psoriasis. Dimethyl fumarate is thought to have immunomodulatory properties without causing significant immunosuppression. Dimethyl fumarate has also been applied as a biocide in furniture or shoes to prevent growths of mold during storage or transport in humid climates. However, due to cases of allergic reactions after skin contact, dimethyl fumarate-containing consumer products are no longer authorised to be manufactured (since 1998) or imported (since 2009) in the European Union. == Epidemiology == The annual incidence is about 1.1 per 100,000 annually in population studies from the United States and France. From 1994 to 2003, the incidence increased threefold; this has been attributed to the more widespread use of modern imaging modalities rather than a true increase. Similarly, those living in urban areas are more likely to receive appropriate investigations, accounting for increased rates of diagnosis in those dwelling in cities. It is suspected that a proportion of cases in people with mild symptoms remains undiagnosed. There is controversy as to whether VAD is more common in men or in women; an aggregate of all studies shows that it is slightly higher incidence in men (56% versus 44%). Men are on average 37–44 years old at diagnosis, and women 34–44. While dissection of the carotid and vertebral arteries accounts for only 2% of strokes (which are usually caused by high blood pressure and other risk factors, and tend to occur in the elderly), they cause 10–25% of strokes in young and middle-aged people. Dissecting aneurysms of the vertebral artery constitute 4% of all cerebral aneurysms, and are hence a relatively rare but important cause of subarachnoid hemorrhage.
