==== Antigen-dependent theories ==== These theories consider a specific combination of antigen peptide sequence and the binding groove (B pocket) of HLA-B27 (which will have different properties from the other HLA-B alleles). === Ebola === After initial studies by the Centers for Disease Control and Prevention (CDC, Atlanta, Georgia, US) in cell culture models, on 6 October 2014, Chimerix received an FDA authorization for emergency investigational new drug applications of brincidofovir for the treatment of Ebola virus disease. Brincidofovir was administered to the first patient diagnosed in the Ebola virus disease outbreak in the US in 2014. The patient was given the drug starting six days after hospital admission when he was already critically ill; he died four days later. Brincidofovir was also given to Ebola patient Ashoka Mukpo at the Nebraska Medical Center, who had developed the disease and then was pronounced Ebola-free and released from the center on 22 October 2014. In October 2014, Chimerix reported it had been given approval by the FDA to start Phase 2 trials in patients infected with ebolaviruses for brincidofovir's safety, tolerability, and efficacy. Organised by a team of scientists at the University of Oxford, including Peter Horby, Jake Dunning, Laura Merson and Trudie Lang, a trial commenced during January 2015 in Liberia, but was subsequently discontinued. Because of a lack of suitable subjects in Liberia, Oxford University and Médecins Sans Frontières planned to extend the trial to Sierra Leone, where there were still Ebola cases; but on 30 January 2015, the manufacturer decided to withdraw support for the trial and end discussion of future trials.
== Risks == The risks are similar to other intraocular procedures, but additionally include graft rejection (lifelong), detachment or displacement of lamellar transplants and primary graft failure. Use of immunosuppressants including cyclosporine A, tacrolimus, mycophenolate mofetil, sirolimus, and leflunomide to prevent graft rejection is increasing but there is insufficient evidence to ascertain which immunosuppressant is better. In a Cochrane review which included low to moderate quality evidence, adverse effects were found to be common with systemic mycophenolate mofetil, but less common with topical cyclosporine A or tacrolimus. There is also a risk of infection. Since the cornea has no blood vessels (it takes its nutrients from the aqueous humor) it heals much more slowly than a cut on the skin. While the wound is healing, it is possible that it might become infected by various microorganisms. This risk is minimized by antibiotic prophylaxis (using antibiotic eyedrops, even when no infection exists). There is a risk of cornea rejection, which occurs in about 10% of cases. Graft failure can occur at any time after the cornea has been transplanted, even years or decades later. The causes can vary, though it is usually due to new injury or illness. Treatment can be either medical or surgical, depending on the individual case. An early, technical cause of failure may be an excessively tight stitch cheesewiring through the sclera. === Early speculation === The possibility of a seventh noble gas, after helium, neon, argon, krypton, xenon, and radon, was considered almost as soon as the noble gas group was discovered. Danish chemist Hans Peter Jørgen Julius Thomsen predicted in April 1895, the year after the discovery of argon, that there was a whole series of chemically inert gases similar to argon that would bridge the halogen and alkali metal groups: he expected that the seventh of this series would end a 32-element period which contained thorium and uranium and have an atomic weight of 292, close to the 294 now known for the first and only confirmed isotope of oganesson. === Inherited Neurodegenerative Diseases === In 2022, a new autosomal-recessive disease was discovered that is caused by mutations of the CTR1 gene. The disease is characterized by profound deficiency of copper in the central nervous system and presents with infantile seizures and neurodegeneration. hCTR1 has been implicated in neurological disorders. Copper imbalance is a hallmark of various neurodegenerative disorders, including Alzheimer’s disease, Parkinson’s disease, where amyloid-β peptides bind copper and generate oxidative stress. Furthermore is's associated with Huntington’s disease, prion diseases, and Amyotrophic lateral sclerosis. The hCTR1-Atox1-ATP7A/B axis is also associated with Menkes disease and Wilson’s disease, in both cases the pathology is driven primarily by mutations in proteins that regulate copper homeostasis, most notably disease-causing mutations in ATP7A or ATP7B, which lead to systemic copper deficiency or copper overload. Moreover, the human eye, and particularly the retina, is composed of highly specialized neurons, and hCTR1 plays a vital role in maintaining copper homeostasis across ocular tissues. === Markers === Myofibroblasts usually stain for the intermediate filament vimentin, which is a general mesenchymal marker, α-smooth muscle actin (human gene = ACTA2), and for palladin, which is a cytoskeletal actin scaffold protein. They are positive for other smooth muscle markers, such as intermediate filament type desmin in some tissues, but may be negative for desmin in other tissues. Similar heterogeneous positivity may exist for almost every smooth muscle marker except probably a few which are positive only in contractile smooth muscles like metavinculin and smoothelin. Myofibroblasts upregulate the expression of fibronectin, collagens, and hyaluronic acid during and after their differentiation from fibroblasts. === Growth over 1997–2010 === India in 1997 allowed foreign direct investment (FDI) in cash and carry wholesale. Then, it required government approval. The approval requirement was relaxed, and automatic permission was granted in 2006. Between 2000 and 2010, Indian retail attracted about $1.8 billion in foreign direct investment, representing a very small 1.5% of total investment flow into India. Single brand retailing attracted 94 proposals between 2006 and 2010, of which 57 were approved and implemented. For a country of 1.2 billion people, this is a very small number. Some claim one of the primary restraint inhibiting better participation was that India required single brand retailers to limit their ownership in Indian outlets to 51%. China in contrast allows 100% ownership by foreign companies in both single brand and multi-brand retail presence. Indian retail has experienced limited growth, and its spoilage of food harvest is amongst the highest in the world, because of very limited integrated cold chain and other infrastructure. India has only 5386 stand-alone cold storages, having a total capacity of 23.6 million metric tons. However, 80 percent of this storage is used only for potatoes. The remaining infrastructure capacity is less than 1% of the annual farm output of India, and grossly inadequate during peak harvest seasons. This leads to about 30% losses in certain perishable agricultural output in India, on average, every year. === Culture === In the past century since its initial discovery, culturing the bacteria in vitro has been difficult. Without the ability to grow and maintain the bacteria in a laboratory setting, discoveries regarding its metabolism and antimicrobial sensitivity were greatly impaired. However, successful long-term cultivation of T. pallidum in vitro was reported in 2017. This was achieved using Sf1Ep epithelial cells from rabbits, which were a necessary condition for the continued multiplication and survival of the system. The medium TpCM-2 was used, an alteration of more simple media which previously only yielded a few weeks of culture growth. This success was the result of switching out minimal essential medium (MEM) with CMRL 1066, a complex tissue culture medium. With development, new discoveries about T. pallidum's requirements for growth and gene expression may occur and in turn, yield research beneficial for the treatment and prevention of syphilis, outside of a host. However, continuous efforts to grow T. pallidum in axenic culture have been unsuccessful, indicating that it does not satisfy Koch's postulates. The challenge likely stems from the organism's strong adaptation to residing in mammalian tissue, resulting in a reduced genome and significant impairments in metabolic and biosynthetic functions. Optimal growth occurs at 33-35 °C, hence infection and lesion development occur on warmer skin areas, but not on colder ones such as the colder ear.
